TI Messenger is the communication solution specified by gematik for the German healthcare system. It enables secure, cross-sector real-time communication between healthcare providers, facilities, and insured individuals. It is comparable to well-known messaging services but complies with the requirements of the telematics infrastructure (TI).

Gematik has chosen the Matrix protocol as its technical foundation: an open, decentralized communication standard developed by the Matrix.org Foundation. Matrix offers end-to-end encryption, a federated architecture, and full interoperability between different providers—features that are essential for the healthcare sector.

What is the Matrix Protocol?

The Matrix Protocol is an open, decentralized communication standard developed by the Matrix.org Foundation. It enables secure, real-time communication with end-to-end encryption and is used by gematik as the technical foundation for the TI-Messenger in the German healthcare system. Its decentralized architecture prevents vendor lock-in and ensures interoperability between different providers.

What advantages does the Matrix protocol offer over proprietary solutions?

The Matrix protocol is an open standard with broad community support. Unlike proprietary solutions, it offers full interoperability, federated identity management, and a decentralized architecture. Of particular relevance to the healthcare sector: There is no central operator with access to communication data, and different TI Messenger providers can communicate seamlessly with one another.

Do I need a gematik certification for my TI Messenger?

Yes. Every TI Messenger that is to be connected to the telematics infrastructure requires approval from gematik. The approval process includes self-conducted tests, interoperability demonstrations, and a security assessment by an accredited assessor.

Can I integrate TI Messenger functionality into my existing app?

Yes, we’ll assist you with the technical integration using the Matrix client-server APIs.

How is the security of communications ensured?

TI Messenger uses the end-to-end encryption provided by the Matrix protocol. Messages are encrypted on the user’s device and can only be decrypted by the recipient. Even the server operator has no access to the content. Since January 1, 2026, the use of elliptic curve cryptography (ECC) has also been mandatory as the encryption technology recommended by the BSI.
